Intake Process

Before scheduling the first appointment, it is required that the client (or legal guardian if the client is under 18 years of age) review and sign the Consent Forms and complete the new client information form.

Adult New Client Information Form

Child/Adolescent New Client Information Form

Once these are received, the clinician will provide their availability to coordinate scheduling the first appointment.
